Position Description

Contract positions of up to 35 hours per week, for a duration of up to 12 months.

Parkdale Queen West Community Health Centre (PQWCHC) is a dynamic, community-based health care organization serving mid-west downtown Toronto. Offering a broad range of services, including primary health care, dental care, harm reduction, health promotion, counselling, and community development programming, we are committed to serving people facing barriers to health care and strive to reduce these barriers. PQWCHC is a multi-sited organization.

General Responsibilities

The Physician provides comprehensive primary health care emphasizing accessibility, health promotion, illness prevention, and continuity of care for our clients, including families, children, refugees, those experiencing homelessness, and other individuals in our urban catchment. The physician works both independently and as part of a multidisciplinary team, primarily based at the Parkdale site.

Reporting Manager

Manager, Primary Care and Allied Health Team

Specific Responsibilities
  1. Providing primary care directly, focusing on accessibility, health promotion, illness prevention, and continuity of care.
  2. Providing primary care indirectly by collaborating with nurse practitioners, consulting with nurses or other staff, and assisting in developing medical or clinical protocols.
  3. Providing on-call coverage on a rotating basis with other physicians to ensure 24/7 medical access throughout the year.
  4. Participating in quality improvement activities.
  5. Acting as a preceptor to medical students and residents, and supporting students from other disciplines.
  6. Providing leadership in the organization and operation of the clinic’s medical activities.
Qualifications
  • Member in good standing of the College of Physicians and Surgeons of Ontario and the College of Family Physicians of Canada.
  • Experience working with clients who identify as 2SLGBTQ+.
  • Experience working with individuals with a history of substance use and/or mental health issues.
  • Ability to work independently and collaboratively within a multidisciplinary team, respecting each member's skills and responsibilities.
  • Understanding of the diverse health and social needs of community members, especially those experiencing poverty and discrimination.
  • Willingness to participate in PQWCHC’s commitment to becoming a discrimination-free health centre.
  • Fluency in languages other than English is an asset.
Compensation

$160.64 per hour plus 6% in lieu of vacation. HOOPP (Health Care of Ontario Pension Plan) option available.

Additional Requirements
