Dream Medical are working with a Community Hospital in Gravesham and are looking for an experienced Band 5 Community Nurse to complete ad-hoc sessions. The session times are 08:00- 16:00 and 09:00 – 17:00 Monday – Friday.

Responsibilities of the nurse:

1. Undertaking complex holistic assessments both in the community and hospital settings
2. Assessing, planning, implementing and evaluating care
3. Working in partnership across role boundaries of health and social care and working within a multi-disciplinary team

Requirements of the nurse:

1. Degree or equivalent in nursing
2. Current NMC registration
3. Significant NHS experience as a nurse

This role offers a lucrative rate of pay that is paid on a weekly basis as well as a one-to-one consultancy.
