Job summary

Lynwood Charlton Centre in Flamborough is seeking a Live in Treatment Team Leader for the Flamborough Live in Treatment Program. This full-time, permanent role reports to the Live in Treatment Program Manager and requires 40 hours per week with flexible hours.

Responsibilities include leading the Live in Treatment team, coordinating care, supervising staff, and ensuring documentation, clinical records, and program delivery meet agency standards. A Class G license and vehicle are required.

Qualifications

• Diploma in CYC or BA in related field required.
• CHYMHs assessment certification preferred or to be obtained within 3 months of hire.
• SMG Crisis Intervention Training required.
• First Aid certification required.

Lynwood Charlton Centre is Hamilton’s Lead Agency for child and youth mental health, providing transformative care through community, residential, and treatment services. We empower young people (prenatal to age 25) and their families with equitable, accessible programs—from counselling and crisis support to skill-building and advocacy. We are currently looking for a Live in Treatment Team Leader for our Flamborough Live in Treatment Program.

FUNCTION

To assume responsibility for the provision of day-to-day care and utilize best practice and evidence informed treatment interventions with children/youth in a Live in Treatment environment. The Live in Treatment Team Leader will perform support the work of the Live in Treatment team through developing effective relationships with caregivers/guardians and service providers in the community. In addition, the Live in Treatment team leader will provide leadership at team meetings, in the community, during staff orientation, and through the oversight of required documentation.

JOB DETAILS

Standard Hours: 40 hours per week working both day and afternoons shifts with the expectation for flexible working hours. On call responsibilities fall outside of typical business hours.

Work Environments: Live In Treatment home, Agency vehicle, Community settings,and familyhomes.

Driving Requirements: G class license with eligibility for F class license, own vehicle used for transporting students and families and minimum 1-million-dollar liability insurance coverage

JOB DUTIES AND RESPONSIBILITIES

As a Staff Person Working Within the Live in Treatment Milieu, Responsibilities Will Include:

• Providing a safe, predictable, and therapeutic environment for the children and youth in the Live in Treatment program
• Ensuring the basic needs of the children and youth in the Live in Treatment program are met
• Planning, developing and implementing individual and group therapeutic programming
• Maintain clinical records, assessments and reports according to agency and program policy and procedures
• Handle, dispense and administer medication according to agency and program policy and procedures
• Will provide leadership while in the role of Shift Coordinator, Acting Building Supervisor in the absence of more senior personnel
• Provide telephone crisis support to caregivers as per individual plans
• Provision of information and education to children/youth, families and community collaterals to support the implementation of the treatment plan
• Providing individual counselling to children/youth

As a Member of the Inter-disciplinary Team, Responsibilities Will Include:

• Attend and participate in team meetings
• Contributes to the development, implementation and evaluation of the Live in Treatment program.
• Contribute to effective team development and processes to promote team cohesion and communication

As a Leader of the Team, Responsibilities Will Include:

Provide leadership and facilitate collaboration with the other members of the interdisciplinary team. Employ a systemic approach to the development of strength-based treatment plans which provide a wide range of evidence informed treatment interventions appropriate to the individual needs, strengths and diversity of the children/youth and families by:

• utilizing relevant intake and service history
• accessing both formal and informal resources
• utilization of standardized assessment recommendations
• regular supervision
• access to consultation

• In the absence of the Manager of Live in Treatment Program, be responsible for:
  • Setting the team meeting agendas and managing the meeting process
  • Represent the agency/program through attendance at community meetings and case conferences
  • Provide support to program staff and liaise with Management when appropriate
  • Responsible to be on-call to the Live in Treatment Program on a rotational basis
  • Oversee planning and implementation of therapeutic program activities and program expenditures
  • Oversee medication policy implementation
• Work collaboratively with the Manager of Live in Treatment Program to ensure that all policies and procedures relevant to case records and expectations as outlined in the Ministry of Children, Community and Social Services (MCCSS) children’s residence licensing requirements are adhered to.
• Review, process, edit and sign off on clinical documentation and incident reports.
• Participate as a member of clinical teams which include intake, implementation, program development and evaluation.
• Will assume additional team and/or agency roles as determined with Manager of the Live In Treatment Program

Other Duties Will Include:

• Contribute to the development of the field by supervising students on placement.
• Other duties as assigned.

TRAINING AND PROFESSIONAL DEVELOPMENT

Participate in all mandatory agency and program training and in-service processes.

Assume responsibility for their own professional development, including keeping up to date with new developments in the field and acquiring new skills to best serve the students and families within the Program.

EXPERIENCE REQUIRED

• Several years of progressive experience working with children/youth in a Live in Treatment setting
• Experience with navigating and/or collaborating with other community systems
• Experience working with and developing intervention plans aimed to enhance children/youth functioning
• Experience collaborating with an interdisciplinary team to set and achieve common goals.
• Experience demonstrating leadership capacity in previous roles

KNOWLEDGE REQUIRED

Knowledge of child and youth mental health and relevant evidence informed treatment interventions

EDUCATION AND TRAINING QUALIFICATION(S)

• Diploma in CYC or Equivalent from recognized community college or a BA in a related field
• Certification in CHYMHs assessment or certification obtained within 3 months of hire

• First Aid
