The Salvation Army Grace Manor is seeking an LPN to perform assessment, planning, implementation and evaluation of resident care, and to provide direction to Health Care Aides. The LPN collaborates with an interdisciplinary team to deliver safe, competent care within regulatory requirements.

The LPN is responsible for the assessment, planning, implementation and evaluation of resident care. The LPN provides support and direction to the Health Care Aides. The LPN is an active member of the interdisciplinary team and is accountable for providing safe and competent nursing care within the scope of LPN practice. All activities carried out by the LPN are consistent with The Salvation Army's mission and values as well as policies, procedures and protocols of SSSR and Grace Manor respectively.
The LPN working at Grace Manor reports to the Unit Manager and must demonstrate knowledge and be familiar with care of mental health clients. The LPN working at Supported Living reports to the Program Manager and must demonstrate knowledge and be familiar with the care of mental health clients. Both programs have 24/7 Licensed Practical Nurse(s) with Health Care Aide(s). A Recreation Therapist and Admin Support are available during the day shift, Monday to Friday. Support is available 24/7 for nursing staff.
