Job summary

Northern Health in Hazelton is seeking a compassionate Licensed Practical Nurse to join the Primary Care team. You will provide high-quality nursing care in settings such as physician practices, homes, schools, and communities, under the supervision of the Primary Care Team Lead or designate.

You will perform assessments, develop care plans with the interprofessional team, and deliver routine nursing care aligned with each client’s goals.

Qualifications

• Active LPN license/registration with BC College of Nurses and Midwives (BCCNM).
• Experience in primary care or community settings (phys nursing) can be an asset.

Responsibilities

In accordance with the established mission, vision, values, and strategic priorities of Northern Health, and under the general supervision of Primary Care Team Lead or designate, the Primary Care Licensed Practical Nurse (LPN) practices in accordance with the standards of professional practice and code of ethics as outlined by the BC College of Nurses and Midwives (BCCNM).

The Primary Care LPN functions as a member of the interprofessional team and applies best practice to provide competent, safe, and ethical care for clients in settings such as physician practices, homes, schools, and communities. Provides care according to the primary care plan developed with the interprofessional team and the client’s personal goals of care. Performs assessments, plans, implements and provides personal and nursing care to clients.

Primary Care Networks are partnerships between the Divisions of Family Practice representing BC’s family physicians, the regional health authorities, the First Nations Health Authority (FNHA) and local community partners, along with other community providers such as nurse practitioners. All partners are working together in their local communities and collectively to redesign B.C.’s primary and community care system so that it works better for patients and health-care providers.
