This position will work in the Haverhill, Newburyport, and Ipswich areas.

Job Purpose

Provide quality nursing care to patients in various community settings. Patient care is delivered in accordance with MGB Home Care's philosophy, policies, and standards of community health practice. Collaborate with the RN case manager to provide skilled nursing visits, ensuring thorough and timely electronic documentation and interdisciplinary communication, in line with agency policies, procedures, and standards of practice.

Qualifications and Experience
  • Massachusetts LPN professional license
  • Minimum 1-year nursing experience in acute or sub-acute care settings
  • Home care experience preferred
  • Effective written, verbal, and electronic communication skills
  • Proficiency with computer software; knowledge of handheld patient care devices preferred
  • Ability to communicate effectively in writing, verbally, and electronically
  • Experience caring for geriatric, pediatric, and/or adult populations
  • Strong interpersonal skills for working with diverse populations
  • Ability to work independently and as part of an interdisciplinary team
  • Strong organizational skills and adaptability to unpredictable circumstances
  • Travel within the local geographic area required
