Job description

JOB TITLE: Licensed Practical Nurse

Site: Meadow Ridge Seniors Village

FTE: 0.00 - Days/Nights

Hours of Work: 06:30-18:30, 18:30-06:30 Rotation - Various

In keeping with the Mission, Vision, and Values of Park Place, the LPN, in collaboration with the interdisciplinary team, uses the nursing process and applies his/her nursing knowledge, skills, and clinical judgment in performing assessments, planning, implementing, and providing quality and person-centered nursing care to the Residents. The LPN operates in accordance with the competency guidelines and full scope of practice as outlined by the relevant provincial governing body’s practice standards and Code of Ethics and Park Place policies and procedures. The LPN practices in a setting where the client care needs are established and changes in health condition are predictable and ensures a commitment to Park Place’s “culture of safety.”

DUTIES AND RESPONSIBILITIES:
  1. Nursing Knowledge
  2. Demonstrates knowledge and the ability to utilize the nursing process in identifying the Residents’ health care needs in an environment that facilitates achieving mutually agreed upon health outcomes;
  3. Uses critical thinking and problem-solving skills in all aspects of care;
  4. Prioritizes and manages Resident care according to identified care needs;
  5. Supports Residents and their families, establishing therapeutic relationships;
  6. Utilizes computer skills for documentation, information sharing, and evidence-based practice validation.

Respects and advocates for Residents’ rights by:

  • Obtaining informed consent prior to nursing procedures;
  • Promoting self-determination and autonomy in care planning;
  • Adhering to Park Place’s “Residents Bill of Rights.”

Understands and applies the Code of Ethics for LPN/RPN to guide decision-making and behavior.

Nursing Practice:
  • Assessment and Care Planning: Utilizes standardized assessment tools (e.g., MDS-RAI), performs comprehensive health and psychosocial assessments, processes orders, documents, reports, provides health teachings, performs medication reconciliation, and participates in care and advance care planning.

Performs and maintains accurate records of controlled medications, reports adverse reactions and medication errors, and evaluates nursing procedures such as assessments, vital signs, activities of daily living, elimination, nutrition, wound care, palliative, and post-mortem care, in accordance with provincial competencies.
