Full Job Description

Our Home Monitoring Program allows individuals to maintain independence with the help of Licensed Practical Nurses (LPNs) who administer medications in patients' residences. (Note: ADL, transferring, or personal care are not within this position's scope.)

Skills and Responsibilities

• Visiting patients in the community and performing assessments (e.g., blood glucose, blood pressure)
• Administering medications (orally, injections, creams, patches)
• Interpreting assessment data to adjust medications accordingly
• Acting in acute situations following company protocols
• Documenting interventions, patient behavior changes, and assessment data
• Submitting daily reports to Clinical Services Managers and pharmacies

Position Requirements

• Driver's License and access to a personal vehicle
• Availability to work some evenings and weekends
• Licensed and practicing with the British Columbia College of Nurses and Midwives (BCCNM)
