ACCOUNTABILITY OBJECTIVE

Under the supervision of the Health and Wellness Manager, the Licensed Practical Nurse performs nursing interventions within their scope of practice, adhering to professional standards and provincial regulations. They assist with implementing policies and procedures related to licensed care.

BACKGROUND / QUALIFICATIONS

Must have a recognized LPN license and be in good standing. Fluency in English (written and verbal) required. Demonstrated leadership and organizational skills, proficiency in computer applications (Word, Excel, MDS/RIA). Minimum of 1 year of experience as an LPN, with some experience in a unionized environment. Resident-oriented with values aligned with Diversicare.

KEY RESPONSIBILITIES

  • Participate in the Verve Quality Improvement (VQI) program.
  • Administer medications according to schedule.
  • Perform treatments and assess residents for changes in condition (e.g., falls, wounds).
  • Update care plans based on residents’ evolving needs; complete documentation for medications and treatments.
  • Communicate effectively with nursing staff regarding resident care and concerns.
  • Refer residents to interdisciplinary team members (e.g., physicians, physiotherapists).
  • Coordinate with departments to ensure quality care and dining experiences.
  • Manage nursing supplies and equipment, notifying supervisors when needed.
  • Supervise and mentor staff, ensuring policy compliance and safety standards.
  • Participate in health and safety activities, including incident reporting, infection control, and emergency procedures.
  • Complete mandatory training and education.
  • Analyze data and develop action plans in collaboration with other departments.
  • Perform additional tasks as delegated by the Registered Nurse.
