Reporting to the Director of Care, the Licensed Practical Nurse (LPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.
Key Responsibilities
Work as a member of the inter-professional team, as described by our Professional Practice Model, applying a person-centered approach to care.
Complete and ensure resident clinical care assessments, care/service plans, and documentation are accurate, up to date, and communicated as required.
Implement and evaluate the effectiveness of care/service plans, making changes as needed.
Assist with scheduling resident care conferences and participate when applicable.
Work closely with the Director of Care to facilitate resident move-ins, move-outs, and transfers.
Provide direct resident care, administer medication/treatments, and document care in compliance with regulatory and Resident Care standards, policies, and procedures.
Process orders, safely store drugs and narcotics, and administer them per regulatory requirements, scope of practice, and policies and procedures.
Facilitate and provide resident/family education.
Plan, coordinate, manage, and evaluate resident care plans in collaboration with the resident/substitute decision maker, physicians, nursing and interdisciplinary teams, and contracted service providers.
Coordinate nursing team assignments to ensure efficient and timely service delivery.
Respond to and manage significant changes in resident conditions or medical emergencies, providing emergency first aid as needed.
Teach, delegate, and direct unregulated care providers.
Coordinate emergency response in the absence of the Director of Care or Director of Health & Wellness/Executive Director.
Address resident and family concerns, ensuring appropriate action is taken or escalating issues to the Director of Care, Director of Health & Wellness/Executive Director, or other departments.
Assist the Director of Care or Director of Health & Wellness in coordinating, implementing, and monitoring Infection Prevention and Control, Health and Safety, Clinical Programs, and Quality/Safety Plans for the Nursing Department.
Work in a position of trust with vulnerable adults (residents).
Uphold and promote the organization's values and philosophy, particularly regarding ethics, morality, and integrity, as outlined in the Code of Conduct.
Perform other duties as assigned.
Qualifications
Current registration as a Licensed Practical Nurse in the applicable Province/State.
Experience working in a senior’s environment and with unregulated care providers preferred.
Education and experience in gerontology, rehabilitation, community, complex, or long-term care an asset.
Strong written and oral communication, critical thinking, care coordination, time management, and customer focus skills required.
Demonstrated sound clinical knowledge.
Current Basic Cardiac Life Support and First Aid certification required.
Computer proficiency and experience with Microsoft Office applications.
Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one.
