Job description

Summary

The Licensed Practical Nurse in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home.

Responsibilities

Clinical Competence

  1. Provides high-quality clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with other healthcare team members, following all infection control procedures. Does not alter the plan of care.
  2. Continuously observes and reports the patient's nursing needs to the supervising RN.

Documentation and Care Delivery

  1. Communicates effectively and timely with physicians, patients, family members, and agency staff regarding patient care issues, visit schedules, and changes, ensuring coordination of care and adherence to laws and regulations.
  2. Documents patient visits thoroughly and accurately, maintaining current medication lists and complying with policy and payer requirements.

Quality

  1. Participates actively in performance improvement initiatives, aiming to reduce hospitalizations and improve patient outcomes.

Professionalism (on time, completes training, compliant)

  1. Attends all meetings punctually, including case conferences, with camera on during video meetings.
  2. Arrives at work with a positive attitude, willing to assist patients and colleagues, and open to learning new skills.

Teamwork (on call, productivity)

  1. Flexible scheduling to provide patient care as needed, including participation in weekend rotations and observed holidays.
  2. Adheres to and participates in the agency's Episode Management process.
  3. Performs other duties as assigned.

Education and Experience

License Requirements

  • Current LPN licensure in the state of practice.
  • Valid driver's license, vehicle insurance, and access to reliable transportation.
  • CPR Certification.
