Job summary

Nurse Next Door is hiring Licensed Practical Nurses for in-home care across Halifax. You will provide professional nursing care, assess client needs, and collaborate with care planners to ensure quality outcomes.

The role emphasizes compassionate care, adherence to provincial nursing scope, and strong communication with clients and the scheduling team. Competitive hourly pay and a supportive, growth-focused environment are offered.

Qualifications

• Current Certificate of Competence and in good standing with the provincial College of Nurses.
• Graduation from a recognized Practical Nurse program.
• 1 year of nursing experience required; three years nursing experience preferred.
• Criminal Record Check with Vulnerable Sector Search (cost covered every 5 years).
• Negative TB test or chest x-ray (cost covered every 5 years).
• Assists clients with activities of daily living and hygiene.

Responsibilities

• Provide excellent customer service and interact tactfully with clients and team members.
• Deliver nursing care per care plan and assessments.
• Review care plans and document accurately before visits.
• Coordinate with Care Designer and Scheduling Team for inquiries and updates.
• Communicate with Care Designer to elevate client experience and care.
• Perform nursing treatments and procedures within provincial scope.
• Educate clients and families about diagnoses and care.

Responsibilities and Duties:

• Excellent customer service skills and ability to deal tactfully with clients, other interdisciplinary team members
• As a brand ambassador the LPN/RPN treats client and team members with courtesy and respect
• Reviews the care plans arranged by the case manager ( Care Designer) prior to beginning a visit, completing records accurately and carrying out all assignments as requested
• Responds to Care Designer and Scheduling Team regarding scheduling inquiries and updates within the agreed upon timeline
• Engages in open communication with your Care Designer when opportunities to elevate your client’s experience and care are identified
• Delivers comprehensive professional nursing care consistent with the care plan, nursing assessments and client needs.
• Perform all skills, treatments and procedures competently in accordance to the provincial scope of LPN/RPN and adheres to the policies, procedures and protocols of Nurse Next Door.
• Assess and provide client and family/caregiver education and information pertinent to diagnosis.

Requirements:

• Current Certificate of Competence and in good standing with the provincial College of Nurses
• Graduation from a recognized program for Practical Nurses and one year of recent related experience in gerontological nursing
• Functions within the limits of own experience and knowledge, and practices safely and competently within the job description.
• 1 year of nursing experience required; three years nursing experience preferred. Community, home health or medical/surgical experience preferred.
• Criminal Record Check with a Vulnerable Sector Search (updated at the nurses cost every 5 years for continued employment)
• Negative TB test or chest x-ray (updated at the nurses cost every 5 years for continued employment)
• Assists clients in all activities of daily living/hygiene assistance, bathing, grooming, linen changes and so forth.
