Your Opportunity:
This is an exciting opportunity to provide quality patient care at the Willow Creek Continuing Care Centre (Long Term Care setting). Come and join the passionate and diverse team at the Willow Creek Continuing Care Centre in Claresholm, Alberta. Nestled in the Porcupine Hills, Claresholm is close to many outdoor activities including camping, hiking, and fishing. Position is in Claresholm, approx. 136km south of Calgary city limits and 80 km North of Lethbridge. Under the management of the Patient Care Manager, the Licensed Practical Nurse (LPN) works as a member of a health care team, collaborating on the delivery of care to meet the needs of patients/clients. The LPN assesses, plans, implements, and evaluates care of patients/clients and work in consultation with Registered Nurses (RNs) and members of the interdisciplinary team to meet the needs of more complex patients/clients. The LPN also directs and provides resident care, supervises and schedules staff as necessary, collaborates with families and residents. As one of the leaders of the front-line team, you will ensure the care needs of residents are being met in a comprehensive and timely manner. The LPN is responsible and accountable to the public for his/her practice, according to the Standards defined by the College of Licensed Practical Nurses of Alberta (CLPNA) Standards of Practice (2003). The place of work is in a community designated for Canada Student Loan forgiveness as outlined in https://www.canada.ca/en/services/benefits/education/student-aid/grants-loans/repay/assistance/doctors-nurses/amount.html. You may be eligible for the Rural Federal Student Loan Forgiveness Program if you meet other eligibility requirements. As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:
Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:
Ability to work as a member of a team and to request and accept direction as required. Demonstrates critical thinking skills, with in scope of practice, including clinical judgment, problem solving skills and strong decision-making ability. Dementia experience is required. Strong written and verbal communication skills are required as is a commitment to integrating Patient and Family Centered Care principles into your practice. The ability to adapt quickly to changing and fluctuating workloads will be vital. A commitment to promoting a positive work environment and to professional development is valued. Preferred Qualifications:
Geriatric experience. Supportive living and/or continuing care experience is an asset. Supportive pathways certification. Non-violent crisis intervention certification. Palliative experience is an asset.
