A leading company is seeking a Health Home Consultant to join their integrated care team. This role focuses on providing education and consultation to support teams and families, ensuring services meet organizational guidelines. The ideal candidate will be a Licensed Clinical Social Worker or Psychologist with experience in the field and excellent organizational skills.
The Health Home Consultant is part of the Health Home person-centered integrated care team. This position provides education to the Health Home team and consults on cases as needed. Provides education and consultation within their professional scope to client support teams and families. In addition, the Health Home Consultant ensures services provided meet organizational guidelines, funder requirements and accreditation standards.
This position fulfills the Specialized Health Consultant role within the DD Health Home program.
Essential Functions:
Works with integrated health care team and Health Home leadership to identify training and support needs for the population served.
Creates and delivers educational series for support staff and families on strategies and resources within professional scope.
Measures success of sessions and adapts resources as needed to increase client/stakeholder satisfaction.
Creates, provides and adapts initial and ongoing trainings for Health Home team members related to strategies to support successful interactions.
Provides consultations on specific cases regarding strategies and available resources for Health Home team members and stakeholders.
Provides consultation and in-services for professionals, organizations, schools or agencies as requested. Attends and contributes to Health Home meetings to improve processes and services. Participates on committees and ad hoc projects as needed.
Reports data on service activities and goals regularly. Creates action plans and future goals as needed.
Documents all services and consultations in Electronic Health Record.
Position Competencies:
Knowledge of assessments, services, treatments and programs for individuals with disabilities, including autism.
Participates in trainings and opportunities for ongoing professional growth and development.
Excellent organizational and time management skills to manage multiple deadlines and meet requirements.
Able to create and deliver engaging educational materials and trainings for a variety of audiences.
Establishes and maintains successful interpersonal relationships with a variety of internal and external stakeholders. Able to work effectively in a team-based environment.
Knowledge of Microsoft Office products and Electronic Health Record systems.
Qualifications:
Must be Licensed Clinical Social Worker or Licensed Clinical Psychologist.
Must have Missouri state license, be in good standing, and maintain current licensure.
Must have at least 2 years of work experience as a LCSW.
Experience working within the Missouri DMH system required.
Must have experience using Microsoft Office suite, including Outlook, Word and Excel.
Previous experience working with Electronic Health Record systems or other data systems preferred.
Additional Requirements:
This position typically works weekday, daytime hours. Occasional evenings and weekends as need.
May travel between program offices including locations around the state, and to community locations to attend trainings and meetings.
Must have current driver’s license and proof of insurance.
Must complete and maintain all required trainings/certifications.
Must maintain confidential information in accordance with HIPAA regulations.
