This role provides leadership to the New West Diabetes Clinic, Tri Cities Diabetes Clinic and the Pulmonary Respiratory Clinic. This role also leads and plans the development of an integrated health system focused on the provision of care for complex medically frail seniors.

The Leader, Local Specialized Community Services Programs (SCSP) provides leadership and oversight to the development, implementation and operationalization of Specialized Community Services Programs within the geographical area and in collaboration with Fraser Health SCSP network and clinical leaders The Leader collaborates and builds strong partnerships with local operational leaders project leads, corporate and support departments, physicians, Division of Family Practice partners, and community partners, always remaining aware of and sensitive to the interests of the various partners. Develops strategies aimed at achieving meaningful health outcomes and quality services that are timely and consistent with Ministry of Health objectives. Oversees communications and knowledge transfer required to ensure effective services across the continuum of care. The Leader is also responsible for providing leadership and management to assigned community clinical teams, as well as overseeing the associated staff and budget.
Responsibilities- Leads the planning, development, implementation and operationalization of Specialized Community Services Programs SCSP within the geographical area, in collaboration with Fraser Health SCSP network, ensuring models of care delivery are meeting the local needs of adults with complex medical conditions and/or frailty and are consistent with Ministry of Health and Fraser Health guidelines.
- Provides direct leadership and management of assigned clinical operation(s). Manages resources and oversees the assigned staff/budget, including operational and administrative activities. Acts as a resource to care teams, provides expert consultative advice to other health care practitioners and acts as a resource for the assigned clinical operation(s) or service contracts. Collaborates and builds strong partnerships to ensure collaboration in the provision of seamless, quality patient transitions. The various partnerships include internal stakeholders such as Long Term Care, Assisted Living, Hospice, Home Health/Support, Respite, as well as external partners such as Primary Care Networks, First Nations, Division of Family Practice.
- Develops, implements and manages a project management framework with templates, samples and guidance including methodology mentorship, in alignment with Fraser Health project management philosophies, that will guide the development and implementation of SCSPs.
- Develops strategies aimed at achieving meaningful health outcomes and quality services that link to key service attributes including service accessibility, appropriateness, acceptability, safety, and efficiency. Participates in strategic operational initiatives, project planning, clinical services development and the development of short and long term tactical clinical, operational or service plans.
- Interprets, applies and ensures operational and service contract plans meet accepted operational and/or contractual requirements, professional standards, collective agreement or other standards and legislated/contractual obligations. Investigates and responds to complaints or grievances. Represents the Department in third party venues.
- Participates in the development and implementation of full complement of evaluation criteria to assess the impact and accessibility of these specialized services. Evaluates services in terms of effectiveness, monitors quality indicators, reviews processes and outcomes, and develops appropriate responses, in collaboration with clinical leaders and partners.
- Leads and facilitates change with a broad range of internal and external stakeholders including physicians, nurses, practitioners, allied health professionals, and other health care providers as well as non-clinical partners.
- Leads communication and knowledge transfer across and within acute sites and community services within the geographical area to ensure effective linkages, sharing of operational plans/clinical objectives, and continuity; provides oversight and direction to nurse educators, nurse clinicians, resource nurses, hospital liaisons, practice leads and project leaders. Manages operating and capital budgets, resources, review of expenses and the development of action plans. Provides leadership of people to foster an environment that is supportive of engagement, innovation, individual and team achievements and the provision of safe, quality care. Acts as an advocate for patients/residents/clients and families; responds to issues in the area of clinical operations to internal and external partners and agencies; ensures compliance of the complaint review and audit process.
- Reviews Fraser Health workplace indicators in areas such as retention, engagement, safety, absenteeism and overtime, and establishes department objectives and action plans.
- Provides leadership for quality care, identifies risks and action plans in terms of clinical/department outcomes and acts as a change agent within the service, fostering an environment of innovation and critical thinking. Recruits people, provides leadership and mentorship on individual leadership and development plans, and provides regular, ongoing, clear and meaningful feedback to provide recognition and to address performance improvement.
- Ensures the Director and leaders of clinical services are informed and aware of significant events and activities within the area of responsibility. Works with leadership to develop long and short-term goals with respect to community-based objectives.
- Participates in the development, implementation and negotiation of essential service contingency plans. Participates in the development and maintenance of administrative and clinical information systems; ensures Fraser Health, government and regulatory reporting requirements are met.
- Ensures Director(s) is/are informed of and aware of significant events and activities in the area(s) of responsibility.
- Participates on internal and external committees and initiatives as assigned.
QualificationsMaster's degree in business or health administration or health care discipline, plus a minimum of seven (7) years recent leadership/management experience, including a minimum of five (5) years in a project or change management role, or an equivalent combination of education, training and experience.
Valid BC Driver's License and access to a personal vehicle for business related purposes.

Professional/Technical Capabilities
- Strong knowledge of project management principles and methodologies coupled with an in-depth understand of the health care system at both the strategic and operational levels within both acute and community settings.
- Demonstrated knowledge and ability in related clinical health care practice, education and program planning, development, implementation and evaluation.
- Knowledge of department management processes under collective agreements
- Demonstrated knowledge of budget planning and preparation principles, concepts and methods.
- Knowledge of health care delivery systems related to specialized community services such as Home Health, Home Support, Adult Day Programs, Long Term Care, Assisted Living, Palliative Care.
- Demonstrated knowledge of budget planning and preparation principles, concepts and methods.
- Proficiency in the use of office software applications as effective tools.
