Ce rôle consiste à soutenir les OHT dans la planification stratégique et la gestion de projets, en mettant un accent particulier sur le développement des équipes de santé et la modernisation des soins à domicile. Qualifications

Ontario Health Central is seeking a dynamic and experienced professional to join our team as the Lead, System Strategy, Planning, Design and Implementation. Reporting to the Director, this role is pivotal in supporting the Integrated Care program areas through strategic planning, project management, and stakeholder engagement. The successful candidate will play a key role in implementing regional priorities, particularly focusing on Ontario Health Team (OHT) development and homecare modernization.

Here is what you will be doing

  • Support OHTs in completing funding deliverables and reports aligned with standardized guidance.

  • Identify and provide additional supports to aid OHTs in deliverable development and performance.

  • Utilize data to drive population-based health planning and support OHTs.

  • Develop and maintain performance-related relationships with OHTs.

  • Advise on healthcare system funding plans and manage related project plans.

  • Identify emerging community issues and develop appropriate planning responses.

  • Monitor implementation and quality improvement activities in healthcare planning.

  • Conduct analyses of local healthcare systems using provincial data and community input.

  • Promote high-quality research and planning methodologies.

  • Lead stakeholder consultations and promote consensus on planning decisions.

  • Contribute to provincial planning activities and institutional knowledge building.

  • Facilitate meetings with partner organizations and public representatives.

  • Foster a culture of integration and coordination within Ontario Health Central.

  • Ensure compliance with Occupational Health and Safety regulations.

  • Support adherence to health service accountability agreements.

  • Coordinate regional initiatives to improve quality and patient safety.

  • Perform other duties as assigned.

This is what you need to be successful:

Education and Experience

  • Bachelor’s degree in health administration, health planning, business administration or a related field with Master-level degree preferred; or an equivalent combination of education and experience in a health care environment

  • Experience in project management required

  • Experience with Home and Community Care Support Services preferred

  • Proficiency with Microsoft Office tools, particularly Excel, Word and PowerPoint required

  • Bilingualism (French and English) an asset

  • Experience with Microsoft Teams and SharePoint an asset

Knowledge and Skills

  • Strong understanding of the Ontario Health Team model and the health system transformation underway in Ontario

  • Strong understanding of Home and Community Care Support Services programs and services

  • Minimum 5 years’ experience in a multi-stakeholder environment, preferably in the health care setting

  • Excellent skills in working diplomatically and collaboratively with partners with different perspectives

  • Good knowledge of and proven ability to apply multiple planning and analysis methodologies and make effective recommendations using a population health management approach

  • Understanding of the current and changing dynamics of the Ontario health-care system

  • Understanding and experience with embedding quality improvement initiatives

  • Excellent communication and interpersonal skills to work effectively with other staff and to liaise with colleagues across Ontario Health and with contacts in the Ministry

  • Must be highly organized and able to thrive in a face paced environment with multiple priorities

  • Uses information to make informed decisions and provides advice; exercises high degree of independence, discretion and professional judgment in problem solving

  • Strong written and verbal communication skills. Internally, communicates with colleagues throughout the organization to present, discuss information and problems related to financial management and funding

  • Externally, communicates directly with various stakeholders to monitor funding allocations and performance

  • Strong attention to detail, demonstrated integrity and professionalism with a solid understanding of financial controls, financial reports and budget management

  • Excellent analytical and problem solving skills required

  • Must be highly organized and able to thrive in a fast-paced environment with multiple priorities.
