Lead Nurse in General Practice - South Petherton & Martock, Somerset

An exciting opportunity has arisen with Symphony Healthcare Services (SHS) to join forward-thinking practices at Martock and South Petherton Surgeries.

As a Symphony Healthcare (SHS) Lead Nurse, you would be part of a nursing team responsible for ensuring that the practice provides optimum care relevant to the practice population, either in practice or the home environment.

You would be able to run a full Practice Nurse clinic. This would include but is not limited to providing general nurse appointment cover and Chronic Disease Management. You will also demonstrate safe, clinical decision-making, assessment, and expert care.

Please note that this post will be based from both South Petherton Medical Centre and Martock Surgery.

This is a permanent post of 30 hours per week, working Monday to Friday. Working pattern to be discussed at interview.

The annual salary for this post is £46,955, plus many excellent employee benefits (see attached poster for more information).

Main duties of the job

As a SHS Lead Nurse you will:

  • Create a development and training plan for the nursing team as a whole, to ensure that the needs of the Practice are covered and all areas have appropriate skill mixes.
  • Maximise the performance of individuals and the efficiency and effectiveness of the team, ensuring the delivery of a high-quality service for patients.
  • Contribute to the development of the practice by participating in all clinical and managerial meetings and helping to construct practice development plans as required.

Qualifications

  • Insight and understanding of current issues in nursing and the NHS.
  • Post-registration study in topics allied to general practice.

Experience

  • Evidence of appropriate knowledge base.
  • Interest and basic knowledge in long-term conditions.
  • Demonstrate an understanding of audit.
  • Experience of working in a General Practice setting.
  • Experience of using a GP Clinical System (EMIS preferred).
  • Basic/advanced certificate of life support.
  • Awareness of the national service framework guidelines.
  • Understanding of health and safety issues.

Knowledge

  • Good interpersonal skills, both verbal and written.
  • Evidence of problem-solving and decision-making skills.
  • Demonstrates motivation, reliability and commitment to team working.
  • Knowledge of needs of patients with long-term conditions.
  • Aware of accountability of own role and other roles in a nurse-led service.
  • Knowledge of health promotion strategies.
  • Awareness of clinical governance issues in primary care.
  • Knowledge of patient group directions and associated policy.
