Qualifications

• Master's Degree in Social Work from an accredited CSWE institution.
• MSW required; LCSW/ACSW required, LCSW preferred (CA). Licensure/LMSW required in states with licensure (TX, TN).
• One year of work experience in a behavioral or healthcare-related setting.

Responsibilities

• Educate patients on treatment options and wellness to enhance quality of life.
• Conduct psychosocial assessments and participate in care planning.
• Support social and emotional well-being of patients and assist with coping with CKD and dialysis.
