Job summary

CMHA Peel Dufferin is seeking an Intensive Case Manager to join the BHSS MDT and provide individualized case management for clients with serious mental health and addictions concerns. The role focuses on attachment to primary care, access to services, and coordinated supports in the community.

Based at our Rutherford Rd S office in Ontario, the position is full-time and on-site. Salary ranges from CAD 62,425 to CAD 73,441 per year, with placement based on qualifications and experience.

Qualifications

  • Post-secondary degree or diploma in relevant discipline (e.g., BSW, MSW, OT, RN).
  • Being a registered and/or regulated health professional is an asset.
  • Minimum 2 years of experience with a focus on assessment, brief interventions and short term supports.
  • Expertise in navigating community mental health and/or community based human services.
  • Minimum of 2 of the following trainings and willing to train up in 6 months: Single Session, CBT, DBT, MI, ACT, Narrative Therapy, Trauma Informed Care, Virtual Care, Interprofessional Multidisciplinary Teams, Crisis Intervention skills.
  • Demonstrated cultural sensitivity and ability to work with Black, African and Caribbean communities.

Responsibilities

  • Conduct needs assessments, develop and implement comprehensive care plans and ongoing monitoring until discharge.
  • Provide contracted case management and activities of daily living related to social determinants of health.
  • Assess and provide crisis support; connect to crisis services when appropriate.
  • Utilize multidisciplinary team to build a collaborative care plan and consult with other disciplines as needed.
  • Manage a caseload including regular re-assessments and risk level updates.
  • Refer clients to community resources and groups supporting recovery goals.
  • Participate in staff meetings and care planning to provide case management expertise within a multi-disciplinary team.
  • Facilitate attachment to primary care providers and timely access to ongoing care.
  • Identify and address barriers to primary care by coordinating referrals and navigation.
  • Follow up with clients and providers to promote continuity of care and maintain connections to primary care.

We are looking for an Intensive Case Manager to join our Black Health and Social Services Multi-disciplinary Team (BHSS MDT)

The Intensive Case Manger will provide individualized case management support to people experiencing serious mental health and addictions concerns. The role will focus on facilitating attachment to primary care, addressing barriers to accessing care, and connecting clients with appropriate health, social and community-based services using a stepped-care approach.

Status: Permanent full-time, 37.5 hours per week. Note, this posting is for an active vacancy.
Salary: $62,425.03-$73,441.21 annually. (Candidate qualifications will be considered when determining the starting salary)
Location: This position is based out of our office at 19 Rutherford Rd S., Ontario. Due tothe nature of the program, this role is on-site and community based.

What You Will Do:
  • Conduct needs assessments, develop, and implement comprehensive care plans and ongoing monitoring until discharge
  • Provide contracted case management, task-based interventions and activities of daily living related to social determinants of health
  • Assess and provide crisis support; make connection to crisis services when appropriate
  • Utilize multidisciplinary team to build a collaborative care plan and consult with other disciplines as needed
  • Manage a caseload including regular re-assessments, updating risk levels and caseload reviews
  • Refer and link clients to community resources and groups that support recovery goals
  • Participate in regular staff meetings and care planning to provide case management expertise for clientcare within multi-disciplinary team
  • Facilitate clients’ attachment and connection to primary care providers, supporting timely access toongoing, comprehensive care.
  • Identify and address barriers to accessing primary care by coordinating referrals, supporting appointmentnavigation and collaborating with BHSS Hub partners and other community providers.
  • Follow up with clients and providers to promote continuity of care and support clients in maintaining their connection to primary care. Facilitate groups, including the creation of new groups as needed and acting as a back-up facilitator
  • Contact with primary care at regular intervals to report clients’ care plans and discharge summary
  • Work collaboratively with other Case Managers to support organizational needs (e.g., volumes across all regions) covered by CMHA PD

  • Demonstrated experience working within a multidisciplinary team and evidence of an understanding of the importance of partnerships and collaboration.
  • Strong interpersonal skills with proven ability to approach, listen, build trust, and communicate across diverse cultures and intersectionality. Willingness to continually improve understanding around cultural competence.
  • Openness to operate from an anti-racist, anti-oppressive, 2SLGBTQ+ affirming and intersectional framework.
  • Willingness to learn and apply practical understanding of different elements of diversity, such as gender identity/expression, race/cultural background, religion, sexual orientation, disability, immigration and precarious status, family status, etc.
  • Fluency in a second language, including ASL (American Sign Language) is considered an asset.
  • Valid Canadian driver’s license and access to an insured automobile in good repair
  • A satisfactory Vulnerable Sector Screening (Police Check) report.
