Intensive Case Management Worker - Community Crisis & Addictions Safety Team

Mission Statement : Homes First Society develops and provides affordable, stable housing and support services to break the cycle of homelessness for people with the fewest housing options.

Position type : Full-time, Permanent

Location : Any Homes First Shelter

Hours of Work : Monday to Friday, 12 pm to 8 pm

Salary : $28.76 / hour

This position supports clients in crisis, who have addictions, and have mental health challenges to access supports, de-escalate situations, and engage in harm reduction strategies, while adhering to the mission statement and policies of Homes First Society.

Job Description :

  • Build relationships with clients with complex needs both inside and outside the shelter.
  • Support clients living rough and at the shelter in accessing shelter services, harm reduction supports, and treatment options.
  • Assist clients in developing harm reduction plans to mitigate overdoses.
  • Collaborate with safe supply clinics, pharmacies, and primary care providers.
  • Help clients access shelters, services, resources, and housing.
  • Conduct perimeter walks around shelters.
  • Accompany clients into the community as needed for support and services.
  • Make referrals to addiction, mental health, and physical health supports.
  • Provide harm reduction education to individuals and groups.
  • Share client information with team members to support client needs and crisis planning.
  • Use assessment tools to develop and monitor support and crisis plans.
  • Respond to conflicts with crisis intervention skills.
  • Work from a Housing First, Harm Reduction, and Trauma-Informed Care philosophy.
  • Visit clients regularly to update assessments and support plans.
  • Be trained to respond to overdoses using oxygen, naloxone, and other tools.
  • Complete documentation and reports as required.
  • Perform other duties as assigned.

Qualifications :

  • Minimum one year of related experience, post-secondary education, or equivalent.
  • Ability to compile information for reports.
  • Experience with case management software.
  • Strong harm reduction skills.
  • Conflict resolution and negotiation skills.
  • Knowledge of mental health and addiction issues, with experience working with affected clients.
  • Experience with individuals lacking primary health care or struggling with medication adherence.
  • Knowledge of harm reduction approaches and referral options.
  • Understanding of the criminal justice system, including court, probation, and police interactions.
  • Professional boundaries and communication skills.
  • Certificate in Homelessness Harm Reduction (preferred).

Assets :

  • Knowledge of group dynamics.
  • Proficiency in Microsoft Office.
  • First Aid, CPR, Crisis Prevention, and Intervention certificates.
  • Ability to work evenings.
  • Experience with homeless populations, diversity, anti-discrimination, and systemic issues related to homelessness.
  • Ability to lift 20 pounds.
