Position Overview

Reporting to the Resource Coordinator, the ISW (Integrated Support Worker) will have knowledge and training in personal care skills and be a certified Health Care Aide (HCA). Preferably, the ISW will also have additional training in working with individuals experiencing mental health or behavioral challenges, which may be obtained after start date. The ISW is responsible for client care involving Health Care Aid duties and supportive care duties as directed by the Case Coordinator/Resource Coordinator. This includes partnering with clients to improve their wellness in community, social functioning, self-care, communication, self-help, and coping. ISWs are located in several housing projects that operate in a 24/7 team model, as well as in the community at large.

Experience
  • Two (2) years’ experience as an HCA (Home Care Attendant / Health Care Aide) preferred.
  • Two (2) years’ experience working in Mental Health preferred (may include experience as an HCA working with clients with complex needs or behaviors).
Education (Degree/Diploma/Certificate)
  • WRHA recognized HCA certificate required.
  • Grade XII minimum or GED equivalent.
Certification/Licensure/Registration
  • Valid driver’s license and vehicle required.
  • May be required to provide a Personal Driver’s Abstract.
Qualifications and Skills
  • Knowledge of:
    • Information and/or training will be provided within the probation period for topics such as PHIA, WHMIS, Principles of Routine Practices, regional and program policies, Mental Health First Aid, SafeTALK, WRHA Violent Prevention modules, and roles of healthcare team members.
  • General skills and ability to perform all duties relevant to the ISW position.
  • Ability to provide safe, effective personal care considering each individual’s capabilities and needs, with respect and privacy.
  • Understanding of client empowerment and challenges faced by individuals with mental and/or physical health issues.
  • Effective communication with clients, family, and coworkers.
  • Ability to assess and teach basic life skills.
  • Flexibility to meet client care needs.
  • Ability to set boundaries and work as part of a team.
  • Strong English verbal and written communication skills.
  • Competence in an Aboriginal language and/or knowledge of Aboriginal customs is an asset.
Physical Requirements
  • Extensive physical activity, including lifting and transferring patients and equipment.
  • Exposure to infectious diseases, blood, body fluids, and other hazards.
  • Potential encounters with aggressive or agitated clients/visitors/staff.
  • Travel between clients in various environments, sometimes under adverse weather conditions.
  • Home environments may include congested spaces and uneven surfaces.

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check, and Adult Abuse Registry Check. The candidate is responsible for any service charges incurred. Checks are valid if obtained within six months prior to employment start date.
