Overview
The Integrated Clinical Specialist in Palliative Care and Oncology was established to lead and advance integrated palliative practice across the province. We are seeking a highly skilled, dedicated, and self-motivated individual to support the delivery of high-quality, evidence-based care. As a key member of our Clinical Nursing Excellence team, this position will focus on providing expert care, guidance, and leadership in both palliative care and oncology. The Clinical Specialist will play a pivotal role in shaping interprofessional best practices across the patient care continuum, from birth through end-of-life, supporting both clinical teams and patients.
This role involves close collaboration with internal and external partners to ensure optimal therapeutic outcomes are achieved. Additionally, the Clinical Specialist will serve as a subject matter expert in skill development, working alongside Clinical Practice Leads and the Learning and Development team to foster ongoing professional growth.
This is a Full-Time position that will include planned travel throughout Ontario as required.

What The Role Involves
- Advance Standards of Care and Professional Practice in Palliative and Oncology Care
- Champion a culture committed to patient-centered, high-quality, and integrated care, and identify opportunities to enable point-of-care excellence in every patient and caregiver interaction.
- Promote and maintain standards of care, best practices, and core competencies in processes and policies supporting palliative care and oncology practice in home and community care.
- Lead the design, implementation, and evaluation of local palliative care and oncology programs aligned with current standards, evidence-based practice, organizational needs, and external requirements.
- Collaborate with interprofessional teams, operations, and leadership to support strategies that improve clinical outcomes from a population health perspective.
- Implement evidence-based care using an integrated care delivery approach, understanding and elevating the roles of all healthcare professionals involved (e.g., Personal Support, Therapies, Nursing).
- Maintain clinical competency in palliative care and oncology.
- Utilize instructional design principles to develop, deliver, and coordinate education in palliative care and oncology.
- Incorporate adult education principles into training programs.
- Develop and implement coaching and mentoring programs to promote best practices.
- Provide expertise to diverse populations in palliative care and oncology.
- Facilitate innovative practices and programming aligned with research initiatives.
- Participate in developing and monitoring quality practice indicators to ensure ongoing fidelity to standards.
- Support strategic priorities related to professional practice and stay informed of emerging trends in technology, healthcare, and adult education.
- Participate in on-call rotations for remote support as part of the clinical practice specialist team.
- Provide high-quality clinical consultation and professional practice support to internal and external stakeholders, including provincial and local leadership, acute care, and health ministries.
- Identify practice gaps within local branches and collaborate with leadership to develop strategies to address these gaps, ensuring alignment with high-quality, patient-centered care.
- Offer strategic advice on palliative care and oncology practices and programs.
Build Capability and Education
- Support leadership and frontline staff with practice and program excellence, ensuring compliance with standards.
- Promote consistency in palliative and oncology practices across branches.
- Collaborate on clinical orientation, role development, and competency building in palliative and oncology care.
- Act as a resource to facilitate full scope of practice for team members.
- Champion digital learning platforms and identify infrastructure needs for responsive learning.
- Support local training and coaching initiatives driven by key performance indicators.
- Assist in developing communication content based on evidence-based clinical and research data, such as emails and newsletters.
- Design and deliver engaging education sessions, both virtually and in person, with proficiency.
Research and Policy
- Identify opportunities for palliative care and oncology research.
- Contribute evidence-based input into policy development and updates for nursing, therapy, and personal support services.
- Participate in conferences, including abstract submissions and speaking engagements.
Special Projects
- Lead and contribute to strategic projects and initiatives from a professional practice perspective.
- Participate in project teams and committees as assigned.
- Perform other duties as required.
What You Bring
- Current registration with the College of Nurses with expertise in palliative and oncology care.
- Master’s Degree in Nursing or progress toward it preferred.
- Over 5 years of healthcare-related leadership experience in home and community care is advantageous.
- Knowledge of OPCN Health Service Delivery Frameworks (Adult Community and Pediatrics, pending).
- Familiarity with current Palliative Care Competency Framework (National and Ontario).
- Understanding of the CHPCA Model to Guide Hospice Palliative Care.
- Knowledge of the Gold Standard Framework and Early Identification Tools.
- Nursing specialization in Palliative Care and Oncology required or expected within 6 to 18 months.
- Certifications in CAPCE, LEAP, Advanced Pain Management, Serious Illness Conversations, CNA, Foundations in Oncology, and Chemotherapy within specified timeframes are preferred.
- LEAP Facilitator certification is an asset.
- Expertise in vascular access care, maintenance, and infusion pump management.
- Knowledge of adult learning principles.
- Valid driver’s license, own vehicle, and insurance.
