Job description

Overview

The Integrated Clinical Specialist in Palliative Care and Oncology was established to lead and advance integrated palliative practice across the province. We are seeking a highly skilled, dedicated, and self-motivated individual to support the delivery of high-quality, evidence-based care. As a key member of our Clinical Nursing Excellence team, this position will focus on providing expert care, guidance, and leadership in both palliative care and oncology. The Clinical Specialist will play a pivotal role in shaping interprofessional best practices across the patient care continuum, from birth through end-of-life, supporting both clinical teams and patients.

This role involves close collaboration with internal and external partners to ensure optimal therapeutic outcomes are achieved. Additionally, the Clinical Specialist will serve as a subject matter expert in skill development, working alongside Clinical Practice Leads and the Learning and Development team to foster ongoing professional growth.

This is a Full-Time position that will include planned travel throughout Ontario as required.

What The Role Involves

  • Advance Standards of Care and Professional Practice in Palliative and Oncology Care
  • Champion a culture committed to patient-centered, high-quality, integrated care and identify opportunities for transformation to enable point-of-care excellence in every patient and caregiver interaction.
  • Promote and maintain standards of care, best practices, and core competencies in processes and policies supporting palliative care and oncology practice in home and community care.
  • Lead the design, implementation, and evaluation of local palliative care and oncology programs aligned with current standards, evidence-based practice, organizational needs, and external requirements.
  • Collaborate with interprofessional patient care teams, operations, and quality and professional practice leadership to support strategies that improve clinical outcomes from a population health perspective.
  • Implement evidence-based care using an integrated care delivery approach, understanding and elevating the roles of all healthcare professionals (e.g., Personal Support, Therapies, Nursing).
  • Maintain clinical competency in palliative care and oncology.
  • Utilize instructional design principles to develop, deliver, and coordinate education in palliative care and oncology.
  • Incorporate adult education principles into training programs.
  • Develop and implement coaching and mentoring to ensure best practice delivery.
  • Provide expertise to diverse populations in palliative care and oncology.
  • Facilitate and champion innovative practices in line with research initiatives.
  • Participate in developing, maintaining, and monitoring quality practice indicators to ensure fidelity.
  • Inform leadership about emerging trends in technology, healthcare, and adult education related to palliative care and oncology.
  • Participate in an on-call rotation providing remote support.
  • Deliver high-quality clinical consultation and professional practice support to stakeholders including local and provincial leadership, acute care, and health agencies.
  • Work with branches to address practice gaps and develop proposals to improve practice and programming.
  • Offer strategic advice on palliative care and oncology practices and programs.

Build Capability and Education

  • Provide consultancy and support to leadership and frontline staff to promote practice and program excellence.
  • Ensure consistency of palliative and oncology practices across branches.
  • Collaborate on clinical orientation, role development, and competency initiatives.
  • Serve as a resource to facilitate full scope of practice for team members.
  • Champion the use of digital learning platforms and support infrastructure development.
  • Support site-specific training and coaching based on key performance indicators.
  • Develop communication content based on clinical and research evidence, such as newsletters and updates.
  • Design and deliver education, with proficiency in large virtual and in-person presentations.

Research and Policy

  • Identify and participate in research opportunities.
  • Contribute evidence-based input into policy development and implementation.
  • Attend conferences, contribute to abstracts, and speak at events.

Special Projects

  • Lead or participate in strategic projects and committees as assigned.
  • Perform other duties as required.

What You Bring

  • Registered with the College of Nurses with expertise in palliative and oncology care.
  • Master’s Degree in Nursing or working towards it preferred.
  • Over 5 years of healthcare experience in clinical practice leadership in home and community care is an asset.
  • Knowledge of OPCN frameworks, Palliative Care Competency Framework, CHPCA Model, Gold Standard Framework, and Early Identification Tools.
  • Nursing specialization in Palliative Care and Oncology required or in progress (within 6-18 months).
  • Certifications such as CAPCE, LEAP, Advanced Pain Management, Serious Illness Conversation, CNA, Foundations in Oncology, Chemotherapy and Biotherapy within specified timeframes.
  • Experience with vascular access care, infusion pumps, adult learning principles, and valid driver’s license with insurance.
