Job summary

CBI Home Health in Peterborough, Ontario, is seeking an Integrated Care Lead (RN) to oversee patient-centered transitions from hospital to home. You will collaborate with hospital teams and families to craft comprehensive care plans, ensuring patients receive the right care at the right time.

Ideal candidates will have 3–5 years of supervisory experience in home and community care, acute care exposure, and strong communication. A valid driver’s license is required.

Qualifications

• Registered Nurse in good standing with the College of Nurses of Ontario (CNO).
• 3–5 years of supervisory experience.
• Experience in home and community care AND acute care.
• Excellent written and verbal communication skills.
• Experience with EPIC will be considered an asset.
• Must have a valid driver’s license and access to a vehicle.

Responsibilities

• Shape seamless, patient-centered transitions from hospital to home.
• Partner with hospital teams, families, and community partners to coordinate care.
• Create comprehensive care plans to support safe discharges and continued home care.
• Ensure frequent, timely communication between staff, patients, families, referral sources and funders.

What you will need to be successful:

• Registered Nurse in good standing with the College of Nurses of Ontario (CNO)
• 3 - 5 years of supervisory experience
• Experience in home and community care AND acute care
• Excellent written and verbal communication skills
• Experience with EPIC will be considered an asset
• Ensure patient and family centric care
• Strong organization skills and able to work in a fast-paced environment
• Ensure frequent, effective, and timely communication between staff and external stakeholders (patients, families, referral sources, funders)
• Be a great team player and work in highly collaborative and interdisciplinary team
• Provide input into business development and/or service delivery model development opportunities
• Possess a valid driver’s license with access to a reliable and insured vehicle
