Intake Social Worker

Department: C & F Services, 927 Clarke Ave. West (Battle Centre), Thornhill, ON L4J 8G6

Employment Type: Full Time

Location: Battle Centre/Day Services/Programs

Reporting To: James Sejjengo

Compensation: $66,000 / year


Description
The Social Worker’s role is to provide information, assessment, and entry facilitation into Reena’s Community Participation Supports and Residential Systems. They maintain contact with individuals who have chosen Reena as their lead agency. The Social Worker plays an integral role in ongoing planning and intervenes in complex cases. Responsibilities include assessment, case management, counseling, service coordination, crisis intervention, advocacy, and outreach. They also develop, lead, and facilitate psychosocial and therapeutic groups.
This position involves participation in high-level committees to liaise and coordinate services within the Developmental Services Ontario (DSO) scope, the Ministry of Health and Long-term Care, and the Central Local Health Integration Network (LHIN). Key Responsibilities
1) Major Responsibilities
  • Manage vacancy systems including DSCIS, DSO lead agencies, and Reena's waitlists and applications.
  • Conduct intake assessments, including screening, liaising with professionals and families, and assessing individual readiness.
  • Draft and submit Support Plans and budgets in collaboration with relevant departments.
  • Facilitate planning and transition processes, including tours and meetings.
  • Coordinate referrals to allied health professionals and financial arrangements.
  • Process admission paperwork and oversee file activation.
  • Work with families on coordination and readiness reviews.
  • Coordinate review meetings and document goals.
  • Collaborate across departments to fulfill intake processes.
  • Perform other duties as assigned.
2) Clinical Responsibilities (Chai-Tikvah)
  • Perform psychosocial and psychological assessments.
  • Develop and implement service and care plans.
  • Provide care coordination, system navigation, and support planning.
  • Assess risk in crises, devise intervention plans, and provide crisis intervention.
  • Offer clinical and supportive counseling using evidence-based approaches.
  • Manage cases over time with tailored care plans.
  • Participate in case conferences and clinical meetings.
  • Advocate for individuals and connect them with community resources.
  • Perform other duties as assigned.
3) System Responsibilities
  • Lead or participate in committee meetings.
  • Stay informed about system changes and trends.
  • Familiarity with supportive housing and intake services.
  • Represent the agency at external meetings.
  • Contribute to staff training and course development.
  • Act as a resource for staff and management.
  • Develop budgets and support plans for residential and community programs.
  • Follow relevant policies and legislation.
  • Coordinate case management and provide clinical guidance.
  • Manage agency service contracts.
  • Perform other duties as assigned.
Skills, Knowledge, and Expertise
1) Qualifications
  • Master’s in Social Work and registration with the College of Social Workers.
  • Annual renewal and good standing with OCSWSSW.
  • At least 2 years of experience in health and developmental services sectors.
  • Knowledge of intake processes and the population served.
  • Experience with families, counseling, and community services.
  • Knowledge of mental health sector is an asset.
