Job Summary

Under indirect or direct supervision and based on understanding patient variables such as age, sex, and other physical characteristics, monitors or manages medication therapy for hospitalized or ambulatory patients; confers with and assists medical personnel in patient care; provides medication information and pharmacokinetic dosing advice; adjusts doses, orders laboratory tests, and administers medications according to policies and procedures; monitors medication distribution; dispenses, compounds, and distributes pharmacy products; provides direction to ancillary staff; and performs other duties as required.

Responsibilities include:

  1. Upholding Kaiser Permanente policies, principles, and applicable laws.
  2. Performing sterile and non-sterile compounding.
  3. Developing and implementing patient-specific medication therapy plans.
  4. Monitoring and optimizing medication therapy based on data and patient outcomes.
  5. Following approved drug therapy management protocols.
  6. Interviewing patients and caregivers to gather medication-related information and documenting it appropriately.
  7. Advising on medication use, storage, and device operation.
  8. Providing pharmacokinetic dosing advice and adjusting medication dosages under protocol.
  9. Reviewing medication orders and ensuring accuracy and adherence to formulary.
  10. Preventing medication errors and reviewing adverse drug events.
  11. Ensuring proper medication storage and participating in investigational studies.
  12. Supporting a collaborative environment and participating in educational and emergency team activities.

Experience and Education Requirements:

  • Minimum of 1200 hours inpatient pharmacy experience or completion of an ASHP-accredited residency.
  • Pharmacy degree meeting licensure requirements.
  • Controlled Substance Registration, California Pharmacist License, NPI, and ACLS/BLS certifications required at hire.

Additional Requirements:

  • Knowledge of patient variables, acute care medications, pharmacological principles, and ability to perform aseptic techniques.
  • Excellent communication skills and knowledge of laws and regulations.
  • Passage of competency assessments or current incumbency in specified job codes.
  • Experience with pharmacy systems and emergency medications.

Preferred Qualifications:

  • Doctorate of Pharmacy (Pharm.D.).
