Job summary

Innomar Strategies, a Cencora company, is seeking a Nurse Case Manager to deliver patient-centered case management, education, and coordination of therapies across Canada. You will work with healthcare providers, manage data, and support program goals.

The ideal candidate holds an RN/RPN/LPN license, has 2–5 years in healthcare or related fields, and is bilingual French/English where required. Travel around 15% may be needed.

Qualifications

• RN/RPN/LPN with active provincial registration is required.
• Minimum two to five years in healthcare, pharma, insurance or related fields is preferred.
• Bilingual French/English is preferable depending on location.

Responsibilities

• Provide patient and healthcare provider support with education on diseases and therapies.
• Coordinate with care teams to manage patient journeys and data collection.
• Follow case-management protocols including data collection, follow-up and adverse event reporting.
• Collaborate with reimbursement specialists and physicians to secure funding for therapies.
• Maintain clear communication with managers, teams and clients regarding patient cases.
• Assist in training new nurse case managers and provide feedback on processes.
