Job summary

Squamish Nation seeks a Wellness Coordinator/Social Worker to support mental health and wellbeing of Squamish Nation members, delivering outreach, counselling, advocacy, and care coordination within the Yúustway Health & Wellness team.

Reporting to the Manager of Kal'númet Primary Care, the role requires a registered clinician experienced in culturally safe practice, with the ability to collaborate with families, healthcare providers, and community partners to navigate complex systems.

Qualifications

  • Master's or Bachelor's Degree in Social Work or related discipline from a recognized post-secondary institution.
  • Registration in good standing with the BC College of Social Workers.
  • Minimum 3 years of recent clinical social work, therapeutic counselling, mental health, substance use, or concurrent disorder experience.
  • 3–5 years of counselling, case management, outreach, system navigation, advocacy, and community-based support services.
  • Experience working in or alongside a First Nations community and understanding Indigenous health and wellness approaches is preferred.

Responsibilities

  • Conduct clinical assessments, treatment planning, case formulation, goal setting, and progress evaluations with clients.
  • Provide education, short-term individual counselling, and group support using culturally safe and harm reduction approaches.
  • Deliver outreach services and build meaningful connections with community members who may be difficult to engage.
  • Complete mental health and substance use intakes and develop individualized Wellness Plans.
  • Provide short-term counselling, consultation, suicide risk assessments, family assessments, and mental health assessments.
  • Participate in interdisciplinary case management meetings and collaborate on care planning for clients experiencing mental health and substance use challenges.
  • Support clients in navigating healthcare systems, including treatment programs, hospital services, specialist appointments, and community resources.
  • Coordinate and support access to temporary or permanent housing resources for clients when appropriate.
  • Collaborate with families, healthcare professionals, Squamish Nation departments, and community partners to provide wraparound support.
  • Support the development and delivery of health education related to mental health, substance use, and concurrent disorders.
  • Maintain accurate client records and documentation within the Electronic Medical Record (EMR) system while ensuring confidentiality and compliance with professional standards.
  • Support community events, program development initiatives, and wellness activities that reflect Squamish Nation culture, values, and traditions.
