Fraser Health Authority is seeking a skilled Mental Health & Substance Use Services liaison for the Primary Care Networks in British Columbia. You will assess, screen, and refer clients, provide brief counselling, and support transitions between PCNs and MH&SUS SCSP teams.

You will also advocate for Indigenous clients, coordinate with community resources, and deliver staff training while maintaining records and adhering to clinical standards.

Qualifikationen

• Master's degree in Social Work, Clinical Counselling or other health-related discipline; eligible for registration.
• Two years' recent experience in community or hospital psychiatry with mental illness and substance use disorders.

• Conduct diagnostic assessments and brief interventions, coordinating treatment plans with MH&SUS and PCN teams.
• Provide short-term counselling and crisis support; liaise with housing, food security and income resources.
• Maintain records, prepare reports, and ensure compliance with regional policies.
