Job summary

Evolve Therapy Services, LLC is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility partner located in Massena, NY. The role spans full time, part time, or PRN arrangements to support resident care and program development.

The position requires a Master’s degree in Speech Language Pathology, CCC, and an active SLP license. Ongoing education to maintain licensure is expected, with growth opportunities within the organization.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency (CCC) required.
• Active SLP license in state(s) of practice is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with target, short-term, and long-term goals.
• Instructs patient, family, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversight and implementation of FEES/MBS protocols and coordination of site programs.
• Participate in screening protocols to meet resident needs timely and with clinical excellence.
