Job summary

Bayshore HealthCare is seeking a Physiotherapist to provide in-home assessment, treatment and education under supervision of Rehabilitation Manager. The role focuses on client-centered physiotherapy interventions in the home setting.

The successful candidate will maintain clinical records, collaborate with the health care team, and ensure discharge planning and goals are documented according to professional standards. Masters in Physiotherapy and current provincial registration are required.

Qualifications

• Masters degree in Physiotherapy.
• Current registration, and in good standing with the provincial College of Physiotherapists.
• Prior experience with in-home assessments and treatments.

Responsibilities

• Assessment of clients, establishment and ongoing evaluation of treatment programs, goals and discharge plans.
• Maintains current clinical records according to professional standards.
• Documents assessment results, goals, treatment, client progress and discharge plans.
• Collaborates with other health care team members as needed.
• Communicates pertinent information and recommendations to clients/care providers.
• Provides education regarding treatment strategies as needed.
• Observes and guides those assisting in the physiotherapy program.
