Job summary

Vancouver Coastal Health (VCH) invites applications for a Community Care Assistant in the Home Support team in North Vancouver, BC. Under supervision, you will provide home support to clients, assist with ADLs/IADLs, and work with families to promote independence within a client-centered care model.

The role emphasizes collaboration with a health care team, safety, and culturally safe care, with expectations to adapt to scheduling changes and meet operational needs.

Qualifications

• Graduation from a recognized Health Care Assistant Program.
• Current/Active Registration with BC Community Health Worker and Care Aide Registry.
• Experience with dementia, geriatric and/or palliative populations is an asset.
• Valid BC driver's license and access to a vehicle (local travel may be required).

Responsibilities

• Provide home support services to clients.
• Assist with activities of daily living (ADLs) and instrumental ADLs (IADLs).
• Collaborate with health care team, clients and caregivers to maximize independence.
• Observe and report significant changes in client presentation.
• Contribute to care planning and motivate clients in their care.
