ROLE PURPOSE

The Home Share Program offers a residential option where adults with developmental disabilities live with a contracted support provider from Connective. The In-Home Caregiver's role is to ensure a safe, supportive home environment where individuals can develop social and life skills, preparing them for independence.

HOURS

This position requires supervision of the client as needed.

LOCATIONS

Coquitlam, Port-Coquitlam, Port Moody, New Westminster, Maple Ridge, Surrey, Delta, Langley, Abbotsford, Chilliwack, Mission, Vancouver, Burnaby

Key Accountabilities

1. Provides flexible and evolving support to individuals with changing needs within a home setting.
2. Works according to the guidelines in the Home Sharing Services Handbook and other relevant policies.
3. Provides suitable accommodation, either owned or leased, as the primary residence of the individual.
4. Ensures a stable, structured, home-like environment with positive role models to guarantee safety.
5. Acts to meet the objectives set for the service user.
6. Cooperates with plans established by the Agency or Ministry for the individual's placement outside the home.
7. Establishes safety and emergency procedures aligned with the Personal Service Plan.
8. Involves the individual and their family in planning and decision-making whenever possible.
9. Assists the individual in making effective choices about relationships and friendships.
10. Maintains regular, professional communication with support teams.
11. Completes administrative duties such as Critical Incident Reports.
12. Reports regularly to the Home Share Manager, including bi-monthly check-ins.

Experience And Skills

• Trauma-informed approach and/or harm reduction model.
• Skilled in conflict resolution and de-escalation.
• Familiar with Positive Behaviour Interventions and Supports.
• Has appropriate space and time to care for the individual.
• Desire or experience in helping individuals achieve independence.
• Significant recent experience caring for individuals facing multiple barriers.

Preference for providers with social services experience, knowledge of non-profit environments, and familiarity with developmental disabilities.

Requirements

• Ability to pass a Criminal Records Check for all residents over 18.
• Current Emergency First Aid Certificate.
• Resides in the Lower Mainland.
• Comfortable working with substance use and mental health issues.
• Basement suite or room in the house is required.

Compensation

Compensation varies by contract and individual, ranging from $1,585 to $3,500 per month.
