Job summary

Winnifred Stewart invites applications for a Support Coordinator who combines direct support to individuals with disabilities with general operations of their residence and community involvement.

You will help develop individualized plans, assist with complex plans, promote rights and choices, provide direct care as needed, monitor health, adhere to medical protocols, build social opportunities, document records, and collaborate with families and staff.

Qualifications

• Diploma or equivalent post-secondary education in a relevant discipline.
• Experience with Autism and Behaviours of Concern (BOC) is an asset.
• Strong written and verbal communication skills.
• Commitment to person-centred planning and community participation.

Responsibilities

• Develop and document individualized, person-centred plans to guide services.
• Assist with support plans in complex situations.
• Encourage individuals to exercise rights and make life choices.
• Provide direct care to high-needs or medically fragile individuals as required.
• Monitor health and well-being and respond to changes.
• Adhere to medical protocols and standing orders for medication and remedies.
• Create recreational and social opportunities within the community.
• Serve as a role model and coach for community participation.
• Maintain clear records and required reports.
• Communicate professionally with individuals, families, staff and the community.
• Work effectively in a team-oriented environment.
