In partnership with Deakin University, WDHS is undertaking a project to implement Heart Failure guidelines in our community. The successful candidate will join a dedicated team working on the I-HEART project, aimed at reducing hospitalisations for patients with heart failure. This two-year project offers the chance to coordinate efforts with local and regional support, engaging clients and service providers through face-to-face assessments and telehealth consultations with Ischaemic Heart Failure specialists.

The successful applicant will need to hold, or be able to obtain, a current National Criminal Police/NDIS Check. Additional checks such as Working with Children, Disability Worker Exclusion Scheme, or NDIS Worker Screening may be required.
