JOB DESCRIPTION:

Relocation service coordination – targeted case management (RSC–TCM), is a Medical Assistance (MA) reimbursed case management service to help people residing in eligible institutions gain access to needed medical, social, educational and other services and supports necessary to move from the institution to a community-based setting.

PRIMARY OBLIGATIONS:

  1. Assessing Person-Served eligibility and determining their housing and service needs.
  2. Assisting Person-Served with effective and efficient discoveries for available and suitable housing.
  3. Developing a customized, Person-Centered housing plan with participants.
  4. Providing support aiding in applying for benefits to increase financial independence, various skills abilities, access to essential community resources, and connecting with various providers.
  5. Responding to Person-Served plans and requests as they arise and working to problem solve and deescalate situations.
  6. Providing community-based Case Management that includes frequent face-to-face and phone contacts with each Person-Served, with a caseload of approximately 5 and/or supporting one's abilities.
  7. Communicating with care team members, community professionals, and other service providers to ensure that services for the Person-Served are properly coordinated and obtained.
  8. Collaborating with government agencies, community organizations, and property owners.
  9. Preparing and maintaining electronic health records, activity documentation, and reports to ensure it is in accordance with the expectations for timely and thorough billing and in compliance with HIPAA and Data Privacy Practices.
  10. Understanding and adhering to program rules and restrictions.

PROGRAM EXPECTATIONS:

  1. Ensuring Person-Served understands their rights and responsibilities as tenants and participants of other programs.
  2. Transporting Person-Served in your personal, insured vehicle for housing and sustaining housing purposes only.
  3. Providing Person-Served with information and referring them to additional support programs or services.
  4. Providing flexible services within a schedule that meets the needs of Person-Served.
  5. Continuously exercising good judgment in handling Person-Served issues.
  6. Representing the organization with a positive and professional image at all times.
  7. Providing and promoting unconditional positive regard to all Person-Served, regardless of race, creed, gender, religion, sexual orientation, disability, and economic status.
  8. Meeting performance criteria as outlined in the annual performance evaluation.
  9. Participating in staff meetings and meeting regularly with program coordinator, manager and/or executive team.
  10. Maintaining current knowledge of housing and community requirements as they develop.
  11. Performing other duties as part of a whole - "TEAM work, make the DREAM work".

EDUCATION AND SKILL QUALIFICATION:

  1. Bachelor's for RSC providers and high school diploma/GED for aides and HSS providers.
  2. Minnesota current and valid driver's license, and an acceptable driving record.
  3. Reliable transportation with valid auto insurance.
  4. The ability to work with integrity, independently.
  5. Knowledge of housing resources, social service agencies, and community services.
  6. Experience working in a diverse continuum of services in a fast-paced, growing agency.
  7. Good verbal and written communication, diverse interpersonal, and active listening skills.
  8. Strong analytical, mediating, and negotiation skills.
  9. Excellent organizational and case management skills.
  10. Complying and retaining a thorough working practice of agency policies, protocols, and procedures.
  11. Leadership skills and the capability of working alone with very little supervision.
  12. Support with innovation and tenacity towards providing creative solutions in supporting individuals in their community safety, with a vision for having a holistic approach.
  13. Ability to understand and operate Microsoft Office programs and software.
  14. Ability to type 45 WPM.

Job Details:

  1. WAGES: $21.00 - $23 per hour, DOQ.
  2. Job Types: Full-time, Part-time.
  3. Salary: From $21.00 per hour.
  4. Willingness to travel: 100% (Preferred).
  5. Work Location: In person.

Education:

  1. Bachelor's Degree (Required).

Experience:

  1. Case management: 1-3 years (Preferred).

License/Certification:

  1. Minnesota Driver License and active insurance (Required).
