Qualifications

• Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
• Completion of an accredited Hospital Pharmacy Residency Program.

Responsibilities

• Provide pharmacotherapy consultation to patients.
• Counsel patients on medication therapies and safety.
• Collaborate with physicians and health care team on treatment plans.
• Assist in development of drug therapy guidelines for conditions.
• Support medication use evaluations and quality improvement initiatives.
• Mentor students, residents and new staff in the department.
• Ensure prescriptions are validated and dispensed accurately.
