Job summary

Fraser Health is hiring a Full-Time Relief Clinical Pharmacy Specialist for the Jim Pattison Outpatient Care & Surgery Centre in Surrey, BC. This role leads evidence-based medication management and clinical guidance for complex cases, with opportunities to teach, research and improve care processes.

Qualifications

• Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
• Certificate of completion of year 1 and year 2 of a Hospital Pharmacy Residency Program accredited by CPRB plus two years' hospital pharmacy experience in the designated specialty area or equivalent.
• Designation in the listed specialty areas (e.g., Anticoagulation, Cardiology, Critical Care, etc.).

Responsibilities

• Provides consultation services to health care professionals and applies advanced theory to assess, plan, implement and evaluate pharmaceutical care in the designated specialty area.
• Offers clinical direction on complex cases, including evidence-based drug therapy evaluation and discharge planning.
• Participates in quality improvement activities, audits and reporting for the program area.
• Provides teaching to undergraduate and postgraduate learners and collaborates with universities on curricula and practicums.
• Leads clinical research activities, data collection and dissemination through journals or conferences.
• Represents the department on multidisciplinary committees and external events.

Education

Doctor of Pharmacy (PharmD) degree

Accredited Hospital Pharmacy Residency

Current registration with the College of Pharmacists of BC
