Position Overview

The Home Care Program provides services to support people in remaining safely in the community. Under supervision of the Manager of Health Services, the Hospital Based Case Coordinator (HBCC) receives referrals, conducts assessments to determine eligibility, and collaborates with clients, families, and interdisciplinary teams to develop, coordinate, and evaluate care plans for discharge from acute care settings. The HBCC also assists in determining if clients' needs are best supported in personal care homes or alternative environments, providing professional intervention as needed, and liaising with healthcare services, hospitals, community partners, and families.

Minimum Qualifications
  • Minimum three years recent nursing experience; hospital-based case coordination experience is an asset.
  • Graduate of an approved Registered Nursing program; BN or BScMH preferred.
Additional Training & Certifications
  • Wound Care & Diabetes courses are assets.
  • Canadian Community Health Nurses Certification (CCHN) or relevant certification is an asset.
  • Proficiency in Procura software and assessment tools like interRAI-HC is preferred.
Licensing & Professional Requirements
  • Maintain registration with the College of Registered Nurses of Manitoba (CRNM).
  • CPR certification as per employer policy.
  • Valid driver’s license and access to a vehicle.

  • Knowledge of nursing standards, ethics, community health practices, and relevant legislation.
  • Proficiency in Windows programs, effective communication, critical thinking, and assessment skills.
  • Ability to coordinate services, prioritize, delegate, and evaluate medical stability for safe discharge.
  • Ability to work in a fast-paced environment, adapt to changes, and function independently or as part of a team.
Physical & Work Environment
  • Availability for days, evenings, weekends; use of a reliable vehicle.
  • Exposure to infectious diseases, physical and emotional stress, and potentially aggressive clients.
Additional Information

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child and Adult Abuse Registry Checks. The employer may consider applicants who do not meet all qualifications if no suitable candidates are found. The role is casual, with in-person work arrangements, and involves 7.75 hours per day, with shifts including days, evenings, and weekends. Application deadline is May 23, 2025.
