JOB SUMMARY
In accordance with the vision and values of the Greater Trail Hospice Society (GTHS), the Greater Trail Hospice Coordinator works to ensure high quality end-of-life care and bereavement support in the Greater Trail area.
A primary responsibility of the Hospice Coordinator is to manage End of Life, Friendly Visitor (non-Nav-CARE trained) and Grief client referrals by assessing clients, matching volunteers and reviewing client needs on an ongoing basis. The Hospice Coordinator works collaboratively with GTHS board members and staff, clients, and community to promote a program that supports the Hospice Palliative Approach philosophy, and CHPCA and BCHPCA standards.
The Hospice Coordinator provides efficient office management to support the Hospice Board and other staff. The Hospice Coordinator provides direction on all administrative duties required to maintain the GTHS program.
Working with the GTHS Board and staff, the Hospice Coordinator is a public ambassador for hospice activities, advocating for increased utilization of our program. The Hospice Coordinator promotes living with the best quality of life possible as health changes and challenges arise, moving towards a supported death.
Duties & Responsibilities
- Represents and promotes GTHS program:
- Maintains the foundational philosophy and standards in all verbal and written interactions within the scope of the Hospice Coordinator role.
- Makes and maintains regular, in-person connections with medical professionals and staff for the purpose of increasing referrals to the hospice program.
- Participates with health care staff in inter-professional Palliative Rounds meetings.
- Participates in local, regional, provincial and national Hospice activities representing the GTHS program as appropriate.
- Oversees Office Management
- Works collaboratively with all GTHS staff and Board to facilitate service operations.
- Monitors and responds to incoming hospice mail, email, and phone messages.
- Manages all office functions including maintaining supplies, library, equipment, etc.
- Provides leadership for the Volunteer Program
- Receives and prioritizes referrals for Hospice Volunteer End-of-life, Friendly Visitor (non-Nav-CARE trained) and bereavement support.
- After assessing client needs, matches clients with Volunteers.
- Maintains the InfoAnywhere client and volunteer database.
- Provides ongoing evaluation and revision of client support plan, collaborating with other team members as required.
- Working with the Board, recruits, screens, interviews, and selects suitable Hospice Volunteer candidates following training.
- Participates in training for all regular and grief Volunteers.
- Working with the Board, organizes regular monthly Volunteer debriefing and education meetings.
- Provides education in the community about GTHS hospice programs:
- Communicates with other service providers about potential supports for clients/patients.
- Supports the Board in providing information/education at community events.
- Maintains a high standard of service and promotes quality improvement strategies in collaboration with the GTHS Board:
- Provides written monthly report to GTHS Board prior to the Board meeting.
- Attends regular weekly staff meetings and team building meetings.
- Provides input to GTHS board on policy and procedure, goals, and evaluation of program.
- Is accountable to the GTHS Board and the GTHS Personnel Committee.
- Remains up to date with all vaccinations and safety requirements of the Interior Health Authority or Ministry of Health, BC.
- Other related duties as negotiated with the Board.
Qualifications
- Proficiency in Microsoft Word, Excel, and other office programs
- Excellent communication skills
- Outgoing, engaging personality
- Self-motivated and able to work independently
- Comfortable working in a fast-paced office environment
- Capable of presenting hospice-related information to the public and healthcare professionals
- Experience in a medical or healthcare setting is an asset
- Experience collaborating with multi-disciplinary teams
- Access to a vehicle for duties outside of the office
