Job summary

Spectrum Health Care is hiring Nurses (RPN/RN) who specialize as Home Support Supervisors to work with clients in Mississauga and surrounding areas (Brampton, Etobicoke). As a Home Support Supervisor, you will lead a compassionate, multidisciplinary team in a community setting, travel to patients' homes, and help shape care delivery in Ontario.

Qualifications

  • CNO registration in good standing is required.
  • Palliative care certification or equivalent training is required.
  • Minimum three years' leadership experience in a community setting.
  • Valid driver's license and access to a reliable vehicle.
  • Excellent English communication skills.

Responsibilities

  • Travel to patients' homes in the community.
  • Collaborate with multidisciplinary teams in a community setting.
  • Assess patient needs and formulate care plans.
  • Manage staff performance including appraisals and progressive discipline.
  • Monitor indicators (caseload, turnover, outcomes) and address feedback and complaints.

We are hiring Nurses (RPN/RN) who specialize as Home Support Supervisors to work with our clients in Mississauga and surrounding areas (such as Brampton and Etobicoke).

Job Description
  • Travel to patient’s homes in the community
  • Work collaboratively with multidisciplinary teams in a community setting
  • Assesses patient needs, according to the timeframe established by contractual agreements, formulates the plan of care and assesses progress towards outcomes
  • Responsible for the management of staff, conducting performance appraisals and implementing progressive disciplinary action
  • Gathers, records and analyzes patient and employee feedback and maintains an awareness of key indicators such as caseload volume, staff turnover, occurrence rates, satisfaction levels, risk levels, and performance appraisal completion rates
  • Ensures that all patient concerns and complaints are addressed and documented in a timely manner
Qualifications
  • Current registration with the Colleges of Nurses of Ontario (CNO) in good standing
  • Successful completion of one or a combination of the following required:
    • Canadian Nursing Association (CNA) certification in Palliative Care Nursing or enrollment in CNA certification program in Palliative Care Nursing; or
    • Learning Essentials Approaches to Palliative and End of Life Care (LEAP); or
    • Fundamentals of Palliative Care
    • The Comprehensive Advanced Palliative Care Education (CAPCE)
  • Minimum three (3) years’ experience with progressive leadership responsibilities as a direct supervisor, preferably in a community setting
  • Demonstrated clinical experience in palliative, hospice, or end-of-life care.
  • Valid driver's license and has access to a reliable vehicle
  • Exceptional interpersonal skills and ability to handle difficult situations in an objective consistent format
  • Excellent English verbal and written communication skills (Additional language skills considered an asset)
  • Solid knowledge of the principles, practices, and methods of community nursing and service delivery to Patients as well as program development, implementation, and evaluation
