Responsibilities

The Home Health Patient Care Manager supervises and coordinates clinical services, providing and directing nursing care to patients in their homes as prescribed by physicians and in compliance with laws, regulations, and agency policies.

• Provides clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
• Receives referrals, assigns clinicians appropriately, evaluates patient orders, and schedules start of care visits.
• Coordinates patient home health benefits, medical necessity, and insurance approvals.
• Ensures patient needs are assessed continuously, and care is individualized, appropriate, and meets eligibility criteria and physician orders.
• Reviews assessments and care plans daily, consulting clinicians as needed.

Education and Experience

Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid Driver's License, vehicle insurance, and access to reliable transportation
