Job summary

CenterWell Home Health, part of Humana, seeks a Home Health Physical Therapist to deliver personalized in-home physical therapy, assess and document progress, and coordinate with physicians and care teams. Responsibilities

• Provide PTAs and Home Health Aides with clear care instructions and oversee performance.
• Document patient observations, interventions and evaluations promptly.
• Report patient progress to physician and care team and submit required documentation and summaries.

By delivering personalized care that focuses on functional improvement, you'll empower individuals to overcome limitations and enjoy a better quality of life. As a Home Health Physical Therapist, you will:

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.

Required Qualifications

• Individuals licensed prior to 2016 would have to meet the educational requirements in place at that time (typically a bachelor's or master's degree).
• Individuals licensed in 2016 or later would have been required to obtain a Doctor of Physical Therapy (DPT) degree to qualify for licensure.
• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Preferred Qualifications

• One year of experience as a Physical Therapist.
• Master's degree in physical therapy.

Additional Information TB Statement

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement

This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
