Responsibilities

The Home Health Patient Care Manager Assistant (LPN) assists the Patient Care Manager in coordinating a team to ensure high-quality care for home health patients in the assigned area.

• Ensures the team provides quality care efficiently, following physician's orders and State/Medicare guidelines.
• Prepares patient lists and necessary documents for weekly case conferences.
• Provides direct patient care as needed, within scope of practice, care plans, and physician orders, coordinating with the healthcare team. The plan of care should not be altered.
• Assists the Patient Care Manager in clinician documentation review and audits to ensure accuracy and completeness of all documentation by field staff.

Education and Experience

License requirements:

• Current CPR certification
• Valid LPN license in MA
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
