Qualifications

• Regulatory body membership in good standing with Ontario colleges (Nurses, Physiotherapists, OT, SOWS, Dietitians, Audiologists & Speech Pathologists)
• 1+ years of community health experience or related field
• Knowledge of health care delivery system and community resources
• Excellent interpersonal, communication, assessment, problem-solving and decision-making skills
• Valid driver’s license and access to a reliable vehicle
• Proficiency with Windows environment and documentation
• Compliance with COVID-19 vaccination policy as a condition of employment

Responsibilities

• Assess care needs and determine eligibility for services in collaboration with patients and families
• Link patients with service providers and coordinate care plan delivery
• Establish a helping relationship with patients and families
• Balance patient needs with available resources while respecting values and preferences
• Coordinate and monitor care plans and engage with a multidisciplinary team
