Who We Are

The Tsleil-Waututh Nation Health and Wellness Department provides wholistic, culturally integrated health services for Indigenous clients and families. Our Home and Community Care Program consists of nurses, home support workers, and administrative support staff. Along with our interdisciplinary Primary Care, Public Health, and Mental Wellness team of family physicians, a naturopath, nurse practitioners, counsellors, Elders, and Registered Nurses, we collaborate and strive to serve the Tsleil-Waututh Nation people through health and wellness services and cultural healing.

Known as the People of the Inlet, the Tsleil-Waututh have lived on the unceded lands of the Burrard Inlet since time immemorial. Entrusted by their ancestors as stewards of the land, water, and air, the Tsleil-Waututh draw on the spirituality and wisdom of their traditional healers and knowledge keepers to fulfill this trust and thereby support the health of their families and communities at large.

Job Summary

The Home and Community Care Workeris an integral member of our Home and Community Care team. In this role you support Tsleil-Waututh Nation community members by maintaining their ability to continue living at home safely. A Home and Community Care Worker assists with clients’ personal care, monitors health changes, performs various housekeeping tasks, meal preparation, grocery shopping and companionship activities for clients in their homes. Some administrative tasks may also be part of this role.

Duties & Responsibilities

Assist with the activities of daily living including but not limited to :

  • Light Housekeeping – including but not limited to vacuuming, dusting, dry mopping, dishwashing, cleaning the kitchen and bathroom, and changing beds.
  • Laundry and linen service, including washing, ironing, and folding clothes.
  • Prepare meals and snacks according to the care plan - this may include following a specialized diet plan as identified on the care plan.
  • Assist with shopping for groceries and clothes.
  • Escort clients on appointments, including doctor visits, friends, family members, or stores.
  • Monitor client supplies.
  • Provide companionship for the client. This includes accompanying clients on walks, trips to social / recreational activities, assistance with hobbies, etc.

Assist with personal care as per care plan including but not limited to :

  • Feeding and mobilization
  • Medication reminders
  • Health advocacy
  • Observe, document, and report changes in the client’s physical condition, behavior, or appearance.

Assist The Home And Community Care Team As Needed

  • Provide administrative support as needed.
  • Participate in team and department meetings as client assignments permit.
  • Provide support to other Community Health programs as needed, i.e. appointment booking.
  • Assist in organizing community health events, i.e. annual flu and COVID19 and outreach clinics, as well as wellness and health promotion events.
  • Collaborate with external health care and social services partners.

Qualifications

  • Health Care Assistant Certificate required
  • Grade 10 or equivalent plus a minimum of one (1) year of related experience in house cleaning, home support, and maintaining healthy environments.
  • May consider alternate combinations of relevant education and work experience.
  • CPR, first-aid, and food safe certifications
  • TB screening
  • A valid BC driver’s license (class 4 preferred) and reliable transportation is an asset.
  • Must successfully pass a vulnerable sector Criminal Record Check
  • Must be fully vaccinated against COVID19 (minimum of 2 doses)
  • Previous experience working in a First Nations community and / or organization is preferred.

You can take directions, work independently, and in collaboration with others. In addition, you have the physical ability to carry out the duties of the position. Flexible, kind, and genuine, you enjoy working in a dynamic environment and can adapt to changing needs and priorities by way of effective organization and time management. You have a strong ethic of client service and are eager to learn about the culture and traditions of diverse Indigenous Nations.

Salary pay-range : $48,289.00 - $62,492.00 - based on qualifications and experience.
