Job Description

The Neighbourhood Home Care Specialist, Integrated Specialty Programs, manages high-performing teams of Nurses, Personal Support Workers (PSWs), and Allied Health professionals, fostering a collaborative, innovative, and learning-focused environment aligned with Spectrum's Mission, Vision, and Values.

You will ensure seamless program operations, using data-driven insights to optimize service delivery, improve quality, support team members to work to full scope of practice including indirect care coordination, and achieve key outcomes. You will work with your care team to ensure patient-centered care that respects the goals of patients, families, and care partners.

This role requires availability Monday through Friday, with participation in On-Call rotations after hours and on weekends.

  • Lead an integrated palliative care team within a designated neighborhood, managing daily service coordination to ensure continuity of care, seamless processes, and an optimal patient and employee experience.
  • Collaborate with your team to assess, plan, and coordinate care with patients, caregivers, and other team members, establishing, reviewing, and revising care plans that support patients to die in their preferred location, respecting patient wishes for MAID if requested.
  • Oversee and monitor indirect care activities provided by regulated and unregulated health professionals on your team.
  • Support your team to deliver high-quality care, including pain and symptom management, advanced clinical skills, end-of-life care, interdisciplinary collaboration, cultural competence, emotional support, ethical and legal understanding, assessment, education, caregiver stress/burden assessment, and advocacy.
  • Build trusting relationships with patients and caregivers, ensuring questions are answered and a seamless care approach is maintained.
  • Manage key internal and external stakeholder relationships effectively.
  • Address quality, safety, and risk issues, develop improvement plans, and participate in quality-of-care reviews with partners.

Qualifications

  • Registered Nurse (RN) in good standing with the College of Nurses of Ontario.
  • Minimum of 5 years of nursing experience in a community setting and 2+ years in a management role.
  • Experience working with palliative care patients.
  • Experience in program development, implementation, and evaluation.
  • Successful completion of one or more of the following:
    • Canadian Nursing Association (CNA) certification in Palliative Care Nursing or enrollment in CNA certification in Palliative Care Nursing;
    • Learning Essentials Approaches to Palliative and End of Life Care (LEAP);
    • Fundamentals of Palliative Care;
    • The Comprehensive Advanced Palliative Care Education (CAPCE).
  • Experience managing teams in a unionized environment is an asset.
  • Experience in care coordination.
  • Valid G driver's license and reliable vehicle.
