Position Type: Full-time Contract Position (35 hours/week – 11 July 2025 start date, with a contract end date of 31 March 2027)

POSITION SUMMARY
CAYR COMMUNITY CONNECTIONS (CAYR) strives to create safe, confidential, and inclusive spaces and services in York Region where people can access dignified support and meaningfully engage in self-determined pathways to well-being. CAYR is the only AIDS Service Organization in York Region and has been operating and serving equity-seeking communities for over 30 years.
We offer specialized and combined programs for people living with HIV/AIDS (newly diagnosed to long-term survivors); people affected by or at risk of HIV and Hep C; people who use drugs; people who identify as 2SLGBTQ+; and people supporting vulnerable and marginalized community members. We are currently recruiting a full-time Hep C Support Program Coordinator for our HCV in York Region: Systems Navigator and Directed Outreach Project funded by the Public Health Agency of Canada . The goal of this project is to reduce the negative health impacts experienced by people who are diagnosed with, or at-risk of, HCV. Through this project, CAYR will establish peer-led initiatives to offer systems navigation, increase access Hep C and other STBBI testing, and strategic community outreach.
JOB DESCRIPTION
• Coordinator will facilitate peer-led participatory advisory committee and support groups for people living with Hepatitis C and promote testing opportunities to diverse York Region communities.
•Coordinator will develop and deliver education sessions and workshops on Hep C for community and organizational audiences, both virtual and in person.
•Develop social media and other communications tools to provide education about Hep C and its prevention.
•Will identify opportunities for intersectional programming, as well as establish and provide linkages to care (system navigation) for individuals who have been diagnosed with Hepatitis C.
This role will include responsibility for:
•Working with priority population groups to develop culturally relevant and appropriate communications and education opportunities
• Working closely with York Region Public Health, other CAYR staff, and sector partners to connect people to Hep C and STBBI testing through intersectional programming
• Offer system navigation expertise, adopting holistic, asset-based approaches and motivational interviewing strategies
•Up-scale and/or intensify opportunities to reach higher risk groups in places where they gather, raising awareness, providing harm reduction and safe sex supplies, and links to testing and support
•Coordinator will be available for navigation of the health care system for individuals who test positive for Hep C, to assist with procurement of medication and family doctors, as well as serve as social support during the process of treatment
• Connecting people to treatment, navigating health insurance and medication coverage
•Work closely with Hep C Support Program Assistant
• Data collection and evaluation
QUALIFICATIONS
• Bachelor’s degree in Social Work, Public Health, Human Services or related disciplines preferred
• Experience and comfort working with marginalized people who are actively using drugs and substances, engaged in sex work, are homeless or street involved, and/or live in tents/camps, youth, newcomers, 2SLGBTQ+ population, individuals with a history of incarceration and individuals experiencing mental health challenges.
· Understanding of and commitment to harm reduction principles.
· Ability to maintain a non-judgmental and non-stigmatizing approach.
· Ability to work from a trauma-informed, anti-discrimination/anti-oppressive lens.
· Understanding of HIV/AIDS, Hep C, drugs and substance use, and social determinants of health.
· Not use homophobic, trans-phobic, sexist, or racist language, or language demeaning to drug users and/or sex workers
· Ability to function calmly and effectively in difficult and/or crisis situations.
· Strong communication and conflict resolution skills.
· Self-awareness and established ability to maintain confidentiality and healthy boundaries.
· Respect for agency policies and protocols.
· Ability to work independently and as part of a team.
· Knowledge of York Region geography and resources an asset.
· Flexibility to work outside regular working hours (evening and weekends)
· A familiarity with York Region, and a comfort traveling independently through York Region.
· Valid Ontario driver’s license, insurance and reliable vehicle is a must
· Experience in the HIV Sector and/or Registered Social worker/Social Service worker with OCSWSSW considered an asset
