Job Description

Client Healthcare of CA requires 1 RN to assist with discharge planning. This is a remote role with an exception allowing the candidate to reside in CA and be a licensed RN in CA.

Work Schedule: M-F, 8:30 AM - 5:30 PM PST

These RNs will work with CA hospitals, SNFs, and other lower-level care entities to safely discharge members to appropriate care levels, utilizing MCG criteria.

Must Have Skills
  • Minimum 5 years of clinical experience as an acute care nurse in med-surg, tele, ICU, or ER
  • At least 2 years (within the 5 years) as a hospital-based discharge planner for discharges to home, SNF, custodial care, LTAC, or ARU, with understanding of services in these settings
  • Strong understanding of Medi-Cal post-acute care benefits
  • Strong understanding of MCG inpatient criteria
  • Experience reviewing facility contracts related to post-acute care levels
  • High accountability in transferring members to appropriate care levels safely and efficiently
  • Compassionate communication skills for member and provider interactions
Day-to-Day Responsibilities

Collaborate with CA hospitals and care facilities to ensure safe discharges to lower care levels.

Experience & Licensure

Minimum 5 years clinical experience as an acute care nurse; minimum 3 years discharge experience; managed care experience.

Licensure: CA licensed RN, residing in CA
