Reporting to the Senior Coordinator, Post-Rehabilitation, the primary responsibility of the Health Program Instructor will be to implement group exercise classes and disseminate education resources throughout all AC Health programs. AC Health programs bridge the gap between clinic to community, providing individuals who have recently been discharged from inpatient/outpatient services with a specialized group exercise program in a community setting. This ensures exercise is person-centered as individuals continue along their therapeutic journey towards optimal health and quality of life. AC Health programs include TIME, RCM, Active Lungs, etc.
The Health Program Instructor will also be responsible for accurately collecting impact data for AC’s research team and reporting on participant progress on a continual basis. The Health Program Instructor will support the Senior Coordinator, Post-Rehabilitation in promoting participation in inclusive activities offered at Abilities Centre, such as fitness, sport, and recreation programs. The Health Program Instructor will collaborate with other departments for next step planning to identify transition pathways that align with participant goals.
